Individuals can apply for a voucher for new eyeglasses through one of our participating social service agencies, a school nurse, or directly to New Eyes. To apply for assistance, complete the application form and mail it and a copy of your recent eyeglass prescription to:

Your application form will be reviewed, and if approved, you will receive a voucher in six to eight weeks. Click here to receive information about the status of your application.
